Valley Health System and Inbound Health Partner to launch Hospital at Home Program

Valley Health System, in partnership with Inbound Health is gearing up to launch a Hospital at Home program, set to launch this summer.

Through the Hospital at Home program patients will receive high-acuity inpatient care in the comfort and safety of their homes – improving patient experience while maintaining the same standards of quality and safety found at The Valley Hospital.

As the United States population ages and the burden of chronic conditions rises, demand for flexible, home-based models has increased. As a result, Hospital at Home programs have gained national momentum and have proven to reduce hospital readmissions and improve patient outcomes.

Eligible patients must visit The Valley Hospital, where their care team will assess their medical condition and home safety protocols. If deemed eligible for the program, the patient’s care team will make a referral to Valley’s Hospital at Home program.

Patients in the Hospital at Home program will receive daily clinical assessments, 24/7 monitoring, medical supplies, and home visits from nurses and clinicians for common inpatient diagnoses, such as pneumonia, congestive heart failure, or cellulitis.

The overall goal of the program is to help patients heal in a comfortable, familiar environment.

The partnership between Valley Health System and Inbound Health leverages Inbound’s virtual and in-market care teams, technology, logistics, and clinical protocols – to enable safe, scalable, and effective care outside of the hospital.
