CSPNJ launches housing supports program for at-risk Medicaid beneficiaries in South Jersey

Collaborative Support Programs of New Jersey (CSPNJ) on Thursday announced that it is officially accepting referrals for its newly deployed Housing Supports Program. Operating as part of New Jersey’s housing services network, the initiative will deliver targeted, time-limited stabilization services to vulnerable residents across six southern counties: Atlantic, Camden, Cape May, Cumberland, Gloucester and Salem.

The expanding program operates in close alignment with the Camden Coalition. Once running at full scale, CSPNJ will possess the organizational infrastructure to support more than 100 individuals concurrently.

Services will be seamless across all five of New Jersey’s Medicaid Managed Care Organizations (MCOs): Aetna®, Fidelis Care, Horizon Blue Cross Blue Shield, UnitedHealthcare, and Wellpoint.

The initiative directly addresses the critical intersection of health care and housing instability. It is funded through a joint grant from the New Jersey Department of Community Affairs (DCA) and the Department of Human Services via the NJ FamilyCare Housing Supports Provider Readiness Program.

To qualify for the zero-cost assistance, individuals must meet three specific criteria:

  1. Insurance: Enrolled in Medicaid/NJ FamilyCare.
  2. Social Risk Factor: Currently experiencing homelessness or dealing with recent incarceration.
  3. Clinical Risk Factor: Diagnosed with a chronic physical health issue or a mental health condition.

The program’s rollout lands at a pivotal moment for the regional behavioral health agency. Across the five community wellness centers that CSPNJ currently runs in Southern New Jersey, a striking 43.57% of active members identify as living in homelessness.

“We’re incredibly grateful to be joining this network of providers to open new opportunities for safe and secure housing for the folks in our communities,” Niki Schlett, CSPNJ deputy director of the Southern Region said. “With the housing epidemic continuing to rise, it is crucial for organizations like CSPNJ and others to be able to show up for those needing assistance in avoiding or recovering from homelessness with a compassionate approach.”

The overarching initiative originally formed in June 2025 under New Jersey’s Section 1115 Medicaid waiver, which redefines stable housing as a foundational social determinant of health. CSPNJ has mobilized a team of six highly trained, dedicated staff members to guide participants through the process.

Though the program is scaling toward a triple-digit capacity, administrators anticipate an immediate, highly focused initial caseload of roughly twenty individuals to ensure personalized care.

The person-centered methodology relies on four primary service pillars to help participants transition into self-sufficiency:

  • Pre-Tenancy Case Management: Conducting housing readiness assessments, navigating complex legal applications, and overcoming systemic barriers to entry.
  • Move-In Supports: Providing logistical and foundational help to establish a physical household.
  • Tenancy Sustainment: Offering ongoing independent living skill development, budgeting assistance, and constructive landlord mediation to ensure long-term lease compliance.
  • Residential Modification and Remediation: Arranging essential physical repairs or adaptations to ensure a participant’s home environment remains safe and accessible.

Incorporated in 1984, CSPNJ has spent more than four decades designing peer-led, values-based programs tailored for individuals navigating behavioral health challenges.
