Human Services Commissioner Sarah Adelman announced on Thursday that New Jersey has reached a historic milestone in senior care, officially expanding the Program of All-Inclusive Care for the Elderly (PACE) to cover every county in the state.
By awarding final funding to specialized organizations, the state has fulfilled a long-standing commitment to ensure that eligible older adults—regardless of their zip code—can access comprehensive, coordinated medical and social services while remaining in their own homes.
The PACE model is specifically designed for individuals aged 55 and older who meet the clinical requirements for nursing home care but prefer to stay in their communities. Each participant is supported by an Interdisciplinary Team (IDT) of professionals who manage everything from primary medical care to social activities and transportation.
“With the right support, older adults can continue living at home and in their communities, which we know can lead to better health, greater independence, and an improved quality of life,” Commissioner Adelman said. “We have fulfilled our commitment to making PACE available statewide.”
To reach 100% statewide coverage, the Department of Human Services identified organizations to serve the final remaining regions:

WelbeHealth was selected to provide coverage for Sussex, Warren and Morris counties.

Senior LIFE has been tapped to serve Hunterdon County.
While the awards are now finalized, the opening of physical centers will be staggered. On average, the process of licensing, construction, and final inspections by state and federal officials takes 18 to 24 months.
The program serves as a “one-stop shop” for senior health, providing all services covered by Medicare and Medicaid without the typical gaps or co-pays. The PACE center acts as a hub for medical care and social interaction, with transportation provided to and from the site.
