Florham Park Rehab & Healthcare launches Chronic Illness Support Model in partnership with HealthDrive

Florham Park Rehabilitation & Healthcare Center announced the launch of a comprehensive Chronic Illness Support model, a new initiative designed to provide holistic, patient-centric care for residents managing long-term health conditions.

In a strategic partnership with HealthDrive, the Morris County-based center will now integrate palliative care, pastoral support, and specialized clinical programming into its daily operations. The program is being spearheaded by Subhra Sen, a board-certified adult-gerontology and primary care nurse practitioner with extensive experience in in-patient medical care.

The new model aims to bridge the gap between clinical treatment and emotional well-being. By combining medical expertise with spiritual and social support, the program focuses on symptom management and stress reduction for patients and their families.

Key features of the Chronic Illness Support model include:

  • Coordinated Palliative Care: Administered by a multidisciplinary team of doctors and specialists.
  • Specialty Integration: Direct support for the facility’s existing Pulmonary and Cardiac Care initiatives.
  • Education & Coaching: Disease-specific health coaching to empower patients in their recovery.
  • Life Enrichment: Connections to community resources and social services to improve overall quality of life.

“Florham Park’s new program for chronically ill patients is truly a next-level model of care,” Erin Studt, regional director of Market Development for Marquis Health Consulting Services, which supports the facility, said. “There is great value in providing a holistic focus on both physical and spiritual wellness.”

The introduction of this support model coincides with a major physical transformation of the facility at 190 Park Avenue. Florham Park Rehabilitation & Healthcare Center is currently undergoing a multi-phase modernization project.

While Phase 1—which focused on remodeling patient areas—is already complete, Phase 2 is set to introduce the largest rehabilitation therapy gym in the greater Morris County region. These upgrades, combined with the new Chronic Illness Support model, position the center as a regional leader in post-hospital and long-term residential care.
